Programs

The variety of community, facility and home-based programs offered by Peak Community Services are coordinated by the Support and Quality Assurance Department.

The SQA Department consists of five QDDP’s (Qualified Developmental Disability Professionals), a Staff Educator, and a Director. Together they:

  1. Serve as an advocate for Peak clients and their rights
  2. Develop and implement Individualized Support Plans and Person Centered Plans
  3. Train Peak staff on clients' unique plans and strategies
  4. Serve as a resource to the community and those seeking services
  5. Provide inter-agency case coordination with State Case Managers and other providers
  6. Monitor and ensure compliance with various Federal, State and CARF regulations
  7. Identify trends from internal data to constantly improve the quality of Agency services

Habilitation Skills

Our Habilitation Skill programming helps men and women with disabilities achieve their dreams through individual and group education. Discover our various courses in healthy living, budgeting, computer skills, social skills, and more!

Supervised Group Living

We offer supervised group living in both our Logansport and Winamac locations. Our skilled direct support professionals provide personal care and community involvement in this 24-hour support program.

Community Living

Our Community Living programming is tailored to the unique wants and needs of every individual. This program helps individuals gain greater independence in their day-to-day lives. Learn how we come along side you in this intentional program!

Family Support Services

We provide both in-home and in-community support to families who have adults or children with developmental disabilities. Learn more about how we can come alongside you to provide care and relief.

Pre-Employment Transition Services

The Pre-Employment Transition Services (Pre-ETS) provides support to students who are between 14 and 22 years old, and are receiving, special education or related services.

Wellness

Wellness is a unique resource that allows individuals access to one-on-one health services with a registered nurse, who builds plans that fit the unique needs of each individual.

Empowering Careers: Peak Community Services

Peak Community Services partners with local businesses to create competitive job opportunities for individuals with disabilities. Our Employment Services Team offers personalized support from job search to on-the-job success, including training, job shadowing, resume help, interview coaching, and ongoing support. Services are free through Vocational Rehabilitation. Explore business partnership opportunities or learn about tax incentives for hiring individuals with disabilities.
